Housing Contract - Parent/Guardian Signature
Students that submit the online portion of their housing contract and thereby agree to the terms and conditions of the housing contract when under 18 years of age must also have a parent/guardian accept the contract terms and conditions using this form. (Students that will turn 18 years old on/before April 15 may affirm their acceptance of the contract without a parent/guardian on/after their birthday on the myHousing Portal.)
Instructions
Complete the following steps by April 15, 2013:
| 1. | Download form below and open in your PDF reader (i.e. Adobe Reader). |
| 2. | Fill out fields on the form in your PDF reader. |
| 3. | Print and sign (student and parent) the form. |
| 4. | Return form using one of these methods: |

| 5. | Summer C admits need to complete the above steps for both their Summer C and Fall/Spring contracts. Summer C form will be available when the Summer C contract goes live on February 12, 2013. |
| If a Parent/Guardian Signature form is not received for a contract period (i.e. Summer C, Fall/Spring) by the date provided above, the student's housing contract may be cancelled. | |
Signature Forms
If student is submitting both the Summer and Fall/Spring contracts, both signature forms must be submitted.
